While pharmaceutical industry involvement in producing, interpreting, and regulating medical knowledge and practice is widely accepted and believed to promote medical innovation, industry-favouring biases may result in prioritizing corporate profit above public health. Using diabetes as our example, we review successive changes over forty years in screening, diagnosis, and treatment guidelines for type 2 diabetes and prediabetes, which have dramatically expanded the population prescribed diabetes drugs, generating a billion-dollar market. We argue that these guideline recommendations have emerged under pervasive industry (...) influence and persisted, despite weak evidence for their health benefits and indications of serious adverse effects associated with many of the drugs they recommend. We consider pharmaceutical industry conflicts of interest in some of the research and publications supporting these revisions, and in related standard-setting committees and oversight panels. We raise concern over the long-term impact of these multifaceted involvements. Rather than accept industry conflicts of interest as normal, needing only to be monitored and managed, we suggest challenging that normalcy, and ask: what are the real costs of tolerating such industry participation? We urge the development of a broader focus to fully understand and curtail the systemic nature of industry’s influence over medical knowledge and practice. (shrink)

The 1964 theorem of John Bell shows that no model that reproduces the predictions of quantum mechanics can simultaneously satisfy the assumptions of locality and determinism. On the other hand, the assumptions of signal locality plus predictability are also sufficient to derive Bell inequalities. This simple theorem, previously noted but published only relatively recently by Masanes, Acin and Gisin, has fundamental implications not entirely appreciated. Firstly, nothing can be concluded about the ontological assumptions of locality or determinism independently (...) of each other—it is possible to reproduce quantum mechanics with deterministic models that violate locality as well as indeterministic models that satisfy locality. On the other hand, the operational assumption of signal locality is an empirically testable (and well-tested) consequence of relativity. Thus Bell inequality violations imply that we can trust that some events are fundamentally unpredictable, even if we cannot trust that they are indeterministic. This result grounds the quantum-mechanical prohibition of arbitrarily accurate predictions on the assumption of no superluminal signalling, regardless of any postulates of quantum mechanics. It also sheds a new light on an early stage of the historical debate between Einstein and Bohr. (shrink)

Indeed… This experience, shared by many during the past three years, and likely multiple times, prompted me to think about my nose. Not since cocaine reentered American mainstream culture (...) in the 1970s and '80s—and found its way into countless movies that reflect the trend—has the nose been so central to a certain state of public life. It's difficult to find a film dealing with American culture from that period and beyond, from Scarface to The Wolf of Wall Street, in which there is not a scene depicting characters snorting the obligatory line or spoon of cocaine. Yes, snorting, sniffing, sneezing, ingesting the line through the nose and finishing leftover dust by collecting it with a moist finger, rubbing it lovingly against the front gum of the mouth. Freud's "Über Coca" looms ominously in the background, the work of a phase in Freud's professional career complicated by his obscure relationships with Parke-Davis (now a subsidiary of Pfizer) and Merck, pharmaceutical companies that supplied him with early samples to tickle his nose (Freud; Merkel). The Sacklers of the moment. In our day, Pfizer wants to transform our noses into defenses against the virus rather than conduits toward mind-altering dreams. In the wake of the Freud and William Halsted cocaine addictions chronicled by Howard Merkel, the irony of the contemporary link between Pfizer and the nose is patent.The nose: that orifice through which the virus creeps, the first stage in breathing, the first filter for air-borne pollutants. Recently, important immunological analyses of COVID have focused on the question of viral load, how much virus is necessary to overcome the nose's filtering function and reach the deeper airways leading to the lungs. The processes triggered by any of the major vaccines prepare antibody blueprints and actual antibodies that can be dispatched to the linings of the nose to counter the virus as soon as the body detects its presence. The Delta, Omicron, and subsequent variants remind us that the virus has mechanisms to challenge this response, namely, increased viral load that intensifies transmissibility and the potential to overwhelm antibodies. Hysteria about the dangers of such incursions is partially a product of biased and often incorrect or incomplete media coverage of the COVID vaccine, which has terrified the vaccinated and fueled the skepticism of the unvaccinated.The virus tries to overpower and confuse the filtering nose in another way, more immediately apparent to our conscious selves. As many as eighty-five percent of COVID infections result in the loss of smell, generally recovered in about two to three weeks in sickened people who overcome the infection. About five percent of COVID victims, however, have not recovered their sense of smell six months or more after they have shed the virus.1 What might it mean to lose one's sense of smell? In our vision-oriented culture, both sound (the ear) and smell (the nose) have been less recognized as primary defense mechanisms than in other cultures. When one hears a sound, the first reaction is to prepare for a threat, before the sonic source is seen or understood. Orientation in the world, knowing from which direction the sound emanates, is key to preparing the body for an encounter with an unknown phenomenon: "To sound (an alarm) means to warn. Isn't any emerging sound understood as a warning? Any unexpected sonic occurrence is seen as an indication of something that is to come and that must be clarified rapidly" (Deshays 34). By the same token, to sample surroundings by sniffing what is in the air signals whether one should inhale or somehow try to hold one's breath in search of cleaner, more palatable air. In the context of stochastic hidden variable theories, Howard has argued that the role of separability—spatially separated systems possess distinct real states—has been underestimated. Howard claims that separability is equivalent to Jarrett‘s completeness: this equivalence should imply that the Bell theorem forces us to give up either separability or locality. Howard's claim, however, is shown to be ill founded since it is based on an implausible assumption. The necessity of sharply distinguishing separability and locality is emphasized: (...) a quantitative formulation of separability, due to D'Espagnat, is reviewed and found unsatisfactory, in that it basically conflates separability and locality in a single notion. We first examine Howard's analysis of the Bell factorizability condition in terms of 'separability' and 'locality' and then consider his claims that the violations of Bell's inequality by the statistical predictions of quantum mechanics should be interpreted in terms of 'nonseparability' rather than 'nonlocality' and that 'nonseparability' implies the failure of spacetime as a principle of individuation for quantum-mechanical systems. We will argue that his argument for the first claim is less than compelling and that any argument (...) for the second claim will be interpretation-dependent and, hence, not generally valid. This paper addresses arguments that “separability” is an assumption of Bell’s theorem, and that abandoning this assumption in our interpretation of quantum mechanics (a position sometimes referred to as “holism”) will allow us to restore a satisfying locality principle. Separability here means that all events associated to the union of some set of disjoint regions are combinations of events associated to each region taken separately.In this article, it is shown that: (a) localised events can be consistently defined without implying (...) separability; (b) the definition of Bell’s locality condition does not rely on separability in any way; (c) the proof of Bell’s theorem does not use separability as an assumption. If, inspired by considerations of non-separability, the assumptions of Bell’s theorem are weakened, what remains no longer embodies the locality principle. Teller’s argument for “relational holism” and Howard’s arguments concerning separability are criticised in the light of these results. Howard’s claim that Einstein grounded his arguments on the incompleteness of QM with a separability assumption is also challenged. Instead, Einstein is better interpreted as referring merely to the existence of localised events. Finally, it is argued that Bell rejected the idea that separability is an assumption of his theorem. Philosophers have recently come to focus on explaining the phenomenon of ​bad beliefs,​ beliefs that are apparently true and well-evidenced but nevertheless objectionable. Despite this recent focus, a consensus is already forming around a particular explanation of these beliefs’ badness called ​moral encroachment​, according to which, roughly, the moral stakes engendered by bad beliefs make them particularly difficult to justify. This paper advances an alternative account not just of bad beliefs but of bad attitudes more generally according to which bad (...) beliefs’ badness originates not in a failure of sufficient evidence but in a failure to respond adequately to reasons. I motivate this alternative account through an analogy to recent discussions of moral worth centered on the well-known grocer case from Kant’s ​Groundwork​, and by showing that this analogy permits the proposed account to generalize to bad attitudes beyond belief. The paper concludes by contrasting the implications of moral encroachment and of the proposed account for bad attitudes’ blameworthiness. (shrink)

It is commonly claimed that Kant's critical philosophy aims to limit reason's speculative use and its metaphysical pretensions. This paper argues that such claims should be amended in light of a technical distinction between negative limits and positive boundaries that Kant held throughout his career. Kant's only extended discussion of this distinction appears in §§57–60 of the Prolegomena, a division entitled “On pure reason's boundary‐determination”. I examine these sections in detail in order to elucidate the account of the limits and (...) boundaries of the activity of different cognitive faculties in the Critique of Pure Reason. I argue that once we understand a key ambition of the Critique to be the positive determination of the boundary between objects of experience and things in themselves, we gain insight into the essential contribution that Kant's theory of the positive utility of ideas of reason makes to the critical philosophy. The wrong kind of reason (WKR) problem is a problem for attempts to analyze normative properties using only facts about the balance of normative reasons, a style of analysis on which the ‘Reasons First’ programme depends. I argue that this problem cannot be solved if the orthodox view of reasons is true --- that is, if each normative reason is numerically identical with some fact, proposition, or state-of-affairs. That’s because solving the WKR problem requires completely distinguishing between the right- and (...) wrong-kind reasons for an attitude. I argue that some facts give both right- and wrong-kind reasons for an attitude. Consequently, no such distinction between the two types of reasons is complete if reasons are facts or the like. I conclude by suggesting that reasons and facts are related by constitution, not identity. (shrink)

One of the book's highlights is its literary approach to theology that at the same time makes room for philosophical exploration of religion. Another is Wettstein's rejection of the usual picture that sees religious life as sitting atop a distinctive metaphysical foundation, one that stands in need of epistemological justification. This book explores and articulates the concepts of the continuous and the infinitesimal from two points of view: the philosophical and the mathematical. The first section covers the history of these ideas in philosophy. Chapter one, entitled ‘The continuous and the discrete in Ancient Greece, the Orient and the European Middle Ages,’ reviews the work of Plato, Aristotle, Epicurus, and other Ancient Greeks; the elements of early Chinese, Indian and Islamic thought; and early Europeans including Henry of Harclay, Nicholas of (...) Autrecourt, Duns Scotus, William of Ockham, Thomas Bradwardine and Nicolas Oreme. The second chapter of the book covers European thinkers of the sixteenth and seventeenth centuries: Galileo, Newton, Leibniz, Descartes, Arnauld, Fermat, and more. Chapter three, 'The age of continuity,’ discusses eighteenth century mathematicians including Euler and Carnot, and philosophers, among them Hume, Kant and Hegel. Examining the nineteenth and early twentieth centuries, the fourth chapter describes the reduction of the continuous to the discrete, citing the contributions of Bolzano, Cauchy and Reimann. Part one of the book concludes with a chapter on divergent conceptions of the continuum, with the work of nineteenth and early twentieth century philosophers and mathematicians, including Veronese, Poincaré, Brouwer, and Weyl. Part two of this book covers contemporary mathematics, discussing topology and manifolds, categories, and functors, Grothendieck topologies, sheaves, and elementary topoi. Cost containment by means of prospective payment and other mechanisms is widely seen as a challenge to modern medicine; but the challenge is seldom articulated clearly in terms of core professional values and the moral content of a claim to professionalism. Medical ethics, as it has evolved as a field of study in the past twenty years, has contributed little to the concept of professionalism in medicine. For an investigation of professionalism in the face of cost containment to evolve fruitfully, (...) several things must occur. The true nature of today's challenge to medicine must be appreciated. The balance between tradition and response to social change in defining a professional value system must be appreciated. Physician income must be explicitly addressed as a moral issue. And practical strategies for investigating and affirming core professional values must be developed for health delivery systems and medical education settings. (shrink)

Employing the National Institute of Mental Health-funded Prevention of Suicide in Primary Care Elderly Collaborative Trial as a case study, we discuss 2 sets of ethical issues: obtaining informed consent for a clinic-based intervention study and using treatment as usual (TAU) as the control condition. We then address these ethical issues in the context of the debate about the quality improvement efforts of health care organizations.
